Alternative Media Request Policy

  1. Student will request alternative media from their Special Services Counselor and at that time will:

    1. Provide their counselor with proof of registration in class(es) for each alternative media item being requested.

    2. Provide their counselor with the title, author, publisher, publishing date, edition and ISBN# for each book being requested in alternative format. 

    3. Provide original receipt(s) or voucher(s) for proof of purchase of book(s) 

      1. EOPS students can receive EOPS voucher(s) from Kathy P.C. (Special Services Rm. 312).

      2. Books can be prepaid for at the College Book Store and picked up when they arrive. 
         

  2. Counselor will:

    1. Determine eligibility format from the student’s assigned accommodations.

    2. Verify registration and proof of purchase.

    3. Read and explain AMC policy and complete request form with the student.

    4. Sign the request form and obtain the student’s signature on the AMC policy and request form.

    5. Copy the request (s) and verification(s), and place them in the student’s file.

    6. Send request and original proof of verifications to the Alternative Media Specialist to process.
       

  3. Alternative Media Center will:

    1. Process the form by first attempting to provide student with the alternative media format initially requested.

    2. If format requested is not available, contact the student, and work with the student and counselor to provide other accommodations.

    3. Contact student when alternative media is available and make necessary appointments for training on use of materials.
       

  4. Student is required to return taped books to the HTC at the end of the semester or the student will not be allowed to register for future classes or receive transcripts.
